In this regard, I hereby declare that:

• I have been provided with the necessary information and have been explained the essential characteristics of the above-mentioned credit product (interest rate, number of fees, credit costs, early repayment conditions, total amount payable), as well as the consequences of failing to comply with the contractual terms;
• The costs included in the total cost of the credit have been explained to me, including the costs of ancillary services related to the credit agreement;
• I was given the opportunity to individually negotiate the contractual terms of the credit agreement I intend to sign. I individually negotiated the origination fee, the monthly fee, and the amount of penalties, and I state that these terms do not place me at a disadvantage, as they are fair in my relationship with the creditor;
• I was asked by the creditor about the likely importance of the information provided and whether any essential circumstances that I expected to be provided in order to make an informed and rational decision regarding the conclusion of the potential credit agreement had been omitted.
• The information provided and the corresponding explanations allowed me, including through my own analysis of other third-party creditors’ offers, to assess whether the proposed credit agreement is suited to my needs and financial situation;
• I am familiar with the content of the contract, all its terms have been explained to me, I have understood and accepted them, and the documents and information underlying the conclusion of the contract are complete and accurate;
• I am familiar with the law and the regulatory acts governing the contract, as well as with the fact that I do not have consumer status;
• I have no ambiguities or misunderstandings regarding the terms of the contract;
• The credit product corresponds to the purpose described above and will be used in accordance with its intended purpose and the economic activity carried out.

Assets procured, produced, constructed, or repaired using the credit funds will be recorded or transferred to the share capital of the enterprise through which I conduct entrepreneurial activity and/or will be used jointly by members or individually, but strictly within the scope of the economic activity carried out.
